As part of our commitment to providing care in an efficient manner, from 1st April 2024 we are changing the way we review our patients who require annual monitoring. The aim of the new system is to proactively invite Patients for a combined review of their conditions in the month of their birth. Apologies if you have recently been reviewed, there will be a transitional period whilst we move to our new system, and we would ask that you please be patient with us.

The overall aim of the NEW recall system is to proactively invite Patients for a combined review of their conditions in the month of their birth. Reviews will be undertaken on an annual basis but Patients with more complex needs or Patients with blood results or a BP outside of the desired target range will need to be seen on an interim basis or until they reach the desired target range.
This should ensure a better awareness of when patients are due for their review and should ensure that patients are not called in unnecessarily several times a year.
You may need to attend for Bloods or provide a Urine sample for analysis prior to your review, so we can discuss the results at your review appointment – our Admin / Reception Team will advise you of this. Failure to attend for bloods, if needed, prior to your review may result in the review being re-scheduled.
Apologies if you have recently been reviewed, there will be a transitional period whilst we move to our new system and we would ask that you please be patient with us, we are trying to make improvements to our processes which will in turn improve the overall experience for you, the Patient.
